Practical Tips for Engaging Children in Telehealth Therapy

Online therapy sessions can be uniquely challenging, especially when working with children. Maintaining engagement and building rapport are crucial components that significantly enhance the therapeutic process. With a little creativity and intention, virtual therapy can be both meaningful and effective.

Here are some practical strategies to help children stay engaged during telehealth sessions:

1. Create a Familiar and Warm Environment

Children may be curious about their new counselor and surroundings. Taking time to create a sense of familiarity can help build trust and comfort.

Try these ideas:

  • Share parts of your own space (e.g., a pet, artwork, or a favorite item in your office).
  • Use a warm, relaxed demeanor.
  • Incorporate expressive non-verbals like hand gestures and vocal fluctuation.
  • Add a touch of fun by dressing up for holidays (e.g., Santa hat, bunny ears, green shamrocks) that the child celebrates.

2. Help the Child Create Their Own Therapy Space

Invite the child to designate a cozy, private spot for sessions. This gives them a sense of ownership and control over their therapeutic environment.

Encourage them to personalize their space with:

  • Favorite pillows or blankets
  • Stuffed animals or comfort items
  • Fidget toys or coloring supplies

  • Emoji Reactions – Use them to express feelings, give feedback, or make check-ins more playful.
  • Chat Box – A great tool for shy kids or to add variety to how they communicate.
  • Virtual Backgrounds – Let the child choose a background that feels fun, safe, or calming. This can even lead to meaningful discussions about what makes them feel comfortable.

4. Use the Whiteboard/Drawing Tool

The whiteboard feature in Zoom offers a versatile way to engage the child creatively and visually. 

Activity ideas:

  • Ask the child to draw their family or a timeline of important life events.
  • Illustrate therapeutic concepts (e.g., draw a stick figure and mark where anxiety is felt in the body).
  • Use it for collaborative drawing or brainstorming.

5. Create Therapy Tools Together in Real Time

Hands-on activities during session time not only enhance engagement but can also provide kids with practical tools they can use independently.

Get crafty with:

  • A “bored box” filled with self-soothing items
  • A decorated therapy journal
  • A self-care collage or vision board
  • A memory box for special items or grief work
